DIRECTIVE 1: Barrier Funds

The Mission:

The Horizon’s Barrier Funds are designed to provide financial resources for projects or programs associated with the four anchors of the Northwest Area Foundation’s Horizon’s program: Community; Leadership; Partnerships; and Poverty Reduction.

Funding Criteria:

Funding will be awarded in support of organizations and programs that pertain to any or all of the four anchors. Funds are not available for businesses or individuals. Proposals that address the anchors with respect to the following values will be prioritized: Include Everyone; Welcome Change; Find and Sustain Hope; Learn Through Sharing. The Elk River Regional Action Corporation [RAC] will prioritize allocations based on the anchors and values, and reserves the right to identify an area of emphasis based on their interpretation of community need.

In addition to addressing the anchors and values, the following priorities will be used to evaluate proposals:

Plan for sustainability

Total project budget

Partners and collaboration

Number of people served

Projects that build capacity

Seed money

Proposals for $1,500.00 or less

Process and Notification:

All proposals will be reviewed by the Elk River RAC, which consists of individuals who have been long term active members of the Horizon’s Planning Committee. Proposals will be reviewed by the RAC and recipients and non-recipients will be notified by RAC within fifteen (15) days after the review meeting.

Deadlines:

Funds will be allocated on a bi-annual cycle. Deadlines for applications will be May 15^th, and November 15^th. Two copies are to be submitted. Funding applications are not to exceed four (4) pages plus the budget.

What kind of grants is not made?

The Barrier Funds normally would not make awards for:

On-going operating support

Fundraising Campaigns

Debt retirement

Lobbying activities

Individuals

Proposal Narrative

Please provide the following information in narrative form n this order. Four pages or less is recommended excluding attachments.

Organizational Information

Brief Summary of your organization and a summary of organization mission and goals.

Description of current programs, activities, strengths, and accomplishments.

Your organization’s relationship with other organizations working to meet the same needs or providing similar services. Please indicate how you coordinate with these organizations

Purpose of Grant

Situation

· The situation- opportunity, problem, issue, need and the community client your proposal addresses.

· How was he need/focus determined?

Specific Activities

· Specific activities for which you are seeking funding

· Who will carry out those activities (describe qualifications)

· Specific objectives or ways in which you will meet goal(s)

· Time frame in which this will all take place

Impact of activities

· How will the proposed activities impact the community?

· How does this relate to the anchors and values?

· How many people will be served?

· Describe the people who will be served (ages, economic status, etc.)

· How will this effort be sustained?

Evaluation

How will you measure the effectiveness of your activities?

What would you consider to be successful outcomes and how will they be identified?

Who will be involved in measuring your outcomes?

Oversight procedures for Grant Awards

Recipients of Grant Awards will be required to:

I. Sign a media/photo and information release. ( You will be agreeing to public viewing of your project award)

II. Submit quarterly reports to Elk River RAC on the progress of the project; until one full year after the project is complete; at that time a Final Report will then be required on the use of the resources. You will be notified in writing along with your award notification of the deadline dates for these reports.(Final report date will be determined at the time of total disbursement of funds awarded)

III. Submit at least two additional points of contact from your Organization

IV. Submit a formal request for the fund amounts being requested; by enclosing an invoice, receipt or other binding document; in writing.

A. Funds can be disbursed totally or partially.

B. Submit who to make the check payable to (we will make payable to whomever you request, however the check will be sent to your organization to disburse accordingly)
